To set up new service, please complete the New Service Application PDF below and email it along with a copy of your closing documents or lease agreement and valid driver license or ID to billing@mmia.co.

Once you have emailed the completed application, signed service agreement, closing documents or signed lease and valid driver license or ID to billing@mmia.co, please call the office at 281-651-1618 to pay the required fees.
